Hallucination and Accuracy Improvements (from upstream Claude Code releases)
- System prompt tool guidance (2.1.21, 2.1.31, 2.1.32, 2.1.33): Progressively improved prompts steer the model toward Read/Edit/Glob/Grep instead of bash equivalents, directly reducing hallucinated file paths and command syntax
- Temperature override fix (2.1.31):
temperatureOverridewas silently ignored in the streaming path — all requests used temperature 1.0 regardless of configuration, directly affecting output determinism - Context window fix (2.1.14): Users were blocked at ~65% context instead of ~98%, forcing premature compaction and loss of grounding context
- Prompt cache invalidation (2.1.30/2.1.33): Fixed cache not invalidating when tool descriptions changed (only checked tool names), causing stale tool descriptions and wrong tool usage
- Phantom content removal (2.1.30/2.1.31): Eliminated "(no content)" text blocks from API conversation history that wasted tokens and confused the model
- Auto-compact timing (2.1.21): Fixed auto-compact triggering too early on models with large output token limits, causing unnecessary context loss
- Subagent fixes: Fixed subagents not inheriting parent model (2.1.19) and unable to access SDK-provided MCP tools (2.1.30/2.1.33)

- Database Reset Functionality: Fixed RESET_DATABASE environment variable not working in production
- Root cause: Database reset logic ran BEFORE tables were created during initialization
- Solution: Moved RESET_DATABASE check AFTER init.sql execution in server/database/db.js
- Now properly deletes from all tables (research_documents, onboarding_documents, company_profiles, users)
- Added comprehensive logging and verification to confirm successful reset
- Enables fresh setup screen instead of login screen on new deployments
- Health Check Endpoints: Fixed Sliplane health check failures causing deployment rejections
